Dieser Umstand hat mich dazu bewegt dieses Thema ausführlich zu beschreiben Im dann folgenden Kapitel beschreibe ich den Einzug von Case Management in die Behindertenhilfe, um anschließend ein praktisches Beispiel anhand der "Assistenzplanung der Ev.Stiftung Alsterdorf" zu geben. Dann fasse ich die Ergebnisse eines Workshops der schleswig holsteinischen Kreise mit dem Thema "Fallmanagement in der Eingliederungshilfe" zusammen. Im nächsten Abschnitt beschreibe ich den Ablauf eines Case Managements und statte das theoretische Grundgerüst, mit zum Teil von mir modifizierten, Instrumenten aus.
